In order to benefit from hospice services, they will present themselves at the Hospice Emanuel Foundation headquarters with the following documents required for registration:\n<br><br>\n* copy of the patient's ID card or passport;\n<br>\n* copy of a recent pension coupon and the patient's CAS insurance certificate\/\nor certificate from the workplace endorsed by CAS;\n<br>\n* copies of medical documents (hospital discharge note, tomography results,\nresult of the histopathological examination).\n<br><br>\nAt the hospice headquarters, the patient or relative will complete an application form for registration. Subsequently, the patient will be visited at home by a doctor and a nurse for the initial assessment of the medical condition and the establishment of the treatment and care plan. The frequency of home care visits will be established together with the patient and the family, depending on the care needs. Depending on the problems identified by the medical team, they may suggest to the patient the intervention of other members of the hospice team (social worker, psychologist, chaplain, volunteer) or
